Transition care packages are funded by the Federal Government and auspiced by Country Health SA. They provide older people with rehabilitation and support services when they move from hospital after an acute illness.
The program assists people to rehabilitate following acute episodes of illness or surgery and aims to assist those people in reaching a lower level of care. The service includes occupational therapy and physiotherapy input and home care.
Transition care is available in a person’s home or at the Lake Terrace site.
Referral for transition care requires an assessment by the aged care assessment team through MyAged care.
